Weigh option
There were no servants at home and we are four siblings.Apart from us there were about 15 servants working at our small mill and we had to feed them too.
We were school going children then,I am talking about the seventies.And mom had to prepare us and send us to school in time.
There were no maids at home,no piped water in the kitchen,no washing machine,no grinder,no kitchen appliances to speak of.
My mother managed all.She did everything-waking the children in the morning,arranging everything including water to bathe as we had no water heaters in the bath,feed them and send them to school.After that the process of taking care of the servants started.She had to feed them in the afternoon, there were fifteen of them mind you!
And when we returned from school she had to feed us all, and on top of it she was the one who looked after our progress in the school as dad used to be busy with his work.
She is in her seventies now and very healthy.'
Consider the present scenario,we have two maids,have all the possible kitchen appliances,piped water,washing machine and what not.
But things do not work as it used to back then.Many a times we find it difficult to look after the progress of our children,we have only two!'
A friend of mine who is in his forties recounted the tale of his family,how things were 25 yrs.ago, not in the nineteenth century .
See what has happened today.We have everything,but almost all the work is done by machines.Earlier we used to walk to school or to work.Today we do not walk anywhere.
There are some small things which we can ourselves do, like doing some washing or cleaning our clothes on week ends,washing our cars,cleaning our rooms at least once or twice in a month,do some things like carrying our files to the boss ourselves in the place of a subordinate staff,carrying our own provisions home,helping the spouse in whatever way we can........
No,we do not do it.
When the family doctor,if you have one,says you have high blood pressure or diabetes then we realize the need to exercise and join a gym or a club.Even then we take a car to reach the place,the distance which if you walked every morning will be exercise enough.
(Does any of us realize the cause of many disorders like hypertension,back ache,obesity,diabetes are indirectly caused by lack of proper exercise early in life?)
Had you done that to begin with you would have probably been healthy after all!
Why are we so dependent on hired labor or machines which, if we avoid using, except in exceptional need,all the problems will not arise at all.Or may at least be postponed until our old age.
Do we hear the bell tolling?

Down the memory lane
It was the mid seventies when he was transfered to Bellary Medical College where I was in my second year of M.B;B.S.We had arranged a reception to him and one of my friends had informed me about Dr.shenoy being a singer.During his reply to our welcome I started shouting from behind a desk(we never wanted to be spotted on the very first day)'sir,a song please'.He coolly looked in the general direction of the shout and started-kaahe ko raar machaye,a song made immortal by K.L.Saigal and we were stunned by his voice.That was the beginning,Dr.Shenoy never forgot me,for he had realised it was me who had shouted and I was under the impression that he never saw me shouting!
Then came the 77 elections.I was an opposer of the internal emergency enforced by Mrs.Indira Gandhi and I plunged into election campaigning not realizing that it was none of my business. I had to finish my studies. The elections took nearly 2 months and I never attended my classes.And then came November 77 and I still remember,it was a Thursday and time for Surgery class by Dr.Shenoy who incidentally was the principal too.At the end of the class he seriously looked at me and said'Mr.Bhat,meet me after the class in my office"I knew the axe was going to fall and it did.
I entered his office and stood like a criminal caught in the act,and he tells me-Mr.Bhat,you do not have sufficient attendence this year and I will be refusing to let you take your examination in December,why did you not attend your classes he asks.I tell him-sir I was not well.'What was wrong?'sir I met with an accident,Where? Sir in Mangalore,So?, Sir I had a fractured leg,'Who treated you?', Sir my uncle,'Where is the X-ray',Sir at home.Says he'Mr. Bhat don't tell me this cock and bull story,I know what is the reason,you went and canvassed during that election.And started his harangue.Said he,'You people,you don't realize it is none of your business to play politics while in college and now see what is the consequence,you can not write your examination.You write your exams in May and you can take your final year in December and let me watch you and see how you manage to do it.That was the last thing any teacher could have told me.It used to be very common for a student to loose that kind of challenge in Medical Colleges those days.A teacher could and did see that a particular student passed or failed.I was quite arrogant and should not have told but told anyway-'sir I will do it'He said 'all the best to you'.Though I secretly hoped he would relent at the last moment,he did not.And I had to take that term exam six months later.
As a matter of record nobody in our college had managed to take pre-final and final year exam in the same year and got away with it.
Under such circumstances it was a routine to inform the parents about the fact.And there used to be a printed format which was filled and sent to the parent.Usually students who faced my predicament wanted to prevent exactly this and were able to get a blank one and send it to parents advising them not to worry if they received similar notice.As I was no exception,I too went to the college office the next day to do just that.To my dismay the concerned clerk showed me a handwritten letter addressed to my father.Our principal had personally written it!Ultimately my father came to know about this.He was so broadminded that he never said a word to me.He was the greatest man I came across in my life.
After a couple of months I was returning to class from the hospital after work(medical students work in the hospital in the mornings and attend theory classes in the afternoons)and right in front was coming our dear Dr.Shenoy from the opposite direction.One thing I have to concede,when it came to admonishing students his language was so flowery and fluent, we had no chance to interrupt(In fact it was very entertaining).'Where are you going?' (Sir,to attend my theory class.)I know how you attend your classes,you people,you attend political meetings,don't attend classes and wander in the corridors,what are you doing here,counting pigs?-I studied in Bellary which is famous for pigs!This went on for 5 minutes and finally he let me go my way.Later I realized he was angry with me because my father had written to him holding the Principal responsible for my behavior(not attending the classes).You know according to Hindu philosophy the Guru is supposed to be like father in GURUKULA(equivalent to a residential school which mine was)
In the year '78 I wrote my final exam,and fortunately I had passed,a feat as I've already mentioned,nobody in the past had done it in our college.
On my first day as intern I was posted to emergency ward,and to my horror our dear Dr.Shenoy was waliking towards me.'what are you doing here he barks'- sir I am on emergency duty.As what?As a houseman.He looked as though he didn't believe me and asked'did you pass bhatta?'and I can never forget the way he extended his hand and shook mine and said 'congratulations,doctor' A lesser mortal would have seen that I never passed the examination but not our dear DR.Shenoy.
One day I asked him why he had rejected a very good offer from a private hospital with a generous package of compensation for which his reply with a mischievous smile was that it was not a teaching hospital. (ಅಲ್ಲಿ ಹುಡುಗ್ರು ಇಲ್ವೋ)When it came to teaching he was a dear!
He was a teacher par excellence,to the core,and without a batch of students to harangue he could never sleep.
An year ago he breathed his last after having done a surgery lasting for three hours at the age of 84 years.
After 1978 there was not a single day with me not remembering him at least once.Every time a student of Dr.Shenoy meets another they have a new issue to recount about him.May God rest his soul in peace.

Children
Many a time we see young parents pampering their children,yes,babies do need pampering but not to the extent of spoiling them.Most of the times it is exactly this.You know a couple gets a much wanted child and it becomes all important to them.Everything is taken care of.The child starts growing,obviously,and the parents start expecting too much.
The most common mistake that the parents do is ignore the child,in a sense that it is only a child and will understand nothing.Wrong,a small baby of about 6 months starts understanding,I know this as only a family physician can.Immunize a baby at 2 months,it will forget.but when you have administered the vaccine at 6th month you are in trouble.The baby then on will recognize you as an enemy!If a baby can have that long a memory imagine what will be happening to the inaction of the parent!The baby will remember!
Never ever give the baby anything unless you are very sure about the need.If the baby asks for something that you can afford and is reasonable do not refuse.On the other hand if the demand is not reasonable never yield.Still better give the child whatever you can afford and that should be reasonable.
Once the child is old enough to understand, explain whatever you can and if you don't know never lie.Children are very smart and will immediately realize if you do.And once you are caught in a lie the trust is gone and you can't regain it.
Eventually the child starts going to school.Remember your childhood.Every parent wants the child to do well,naturally.But judge your child's capacity.Do not have unrealistic expectations.contemporaries of your child may be scoring well and your child unfortunately may not.Remember, the education is not made to order.One child may have the aptitude for the subjects taught and another not.But in the prevailing circumstances there is nothing that can be done, until university level.Do not lose heart,every child can not become a doctor or engineer and need not be and nothing is lost.Ultimately what is important is that the child should become responsible citizen of the country.
I am not arguing that academic success is not important,but if unfortunately a child is not very good academically all is not lost.
Remember that Bill Gates is a college drop out and so is many important people in the society.
Usually parents want their children to achieve what they wanted and could not.They want their children to fulfill their dreams. Your children can't be you a couple of decades after your childhood.
and never ever decide or dictate to the child what it should do or not do.Let it decide as the generation today is smarter than what it was a couple of decades ago,they have their own wishes,likes and dislikes.
If you decide what your child is going to do or not do you will have a child no better than you!

ನಾನಾಗ ವೈದ್ಯಕೀಯ ವ್ಯಾಸಂಗದ ಅಂತಿಮ ವರ್ಷದಲ್ಲಿದ್ದೆ,ರಜೆಯಲ್ಲಿ ಊರಿಗೆ ಬರುತ್ತಿದ್ದವ ನನ್ನೆದುರಿಗೆ ಸಿಕ್ಕಿದ ನನ್ನ ಸಂಬಂದಿಕ ಇಂಜಿನಿಯರಿಂಗ್ ವಿದ್ಯಾರ್ಥಿಯನ್ನು ನೋಡಿ ಅಲ್ಲೇ ನಿಂತೆ.ಆರು ತಿಂಗಳಲ್ಲಿ ಹತ್ತು ವರ್ಷ ಪ್ರಾಯ ಜಾಸ್ತಿ ಆದವರಂತೆ ಕಾಣುತ್ತಿದ್ದವನನ್ನು ವಿಚಾರಿಸಿದಾಗ ಸಿಕ್ಕಿದ ವಿಷಯವಿಷ್ಟು.ಆರು ತಿಂಗಳ ಹಿಂದೆ ಅವನಿಗೆ ಜ್ವರ ಶುರುವಾಗಿತ್ತಂತೆ.ಆಗ ನಮ್ಮ ಪರಿಸರದಲ್ಲಿ ಒಬ್ಬ ಹೋಮಿಯೋಪತಿ ವೈದ್ಯರಿದ್ದರು.ಅವರಲ್ಲಿ ಚಿಕಿತ್ಸೆ ಪಡೆಯುತ್ತಿದ್ದವರಲ್ಲಿ ಈ ಹುಡುಗ ಒಬ್ಬ.ಅವರೇನೋ ಹೇಳಿದ್ದರಂತೆ ಈ ಕಾಯಿಲೆ ವಾಸಿ ಆಗದ್ದು,ಇನ್ನು ಬದುಕುವ ಸಾಧ್ಯತೆ ಕಡಿಮೆ ಅಂತ.ಅದನ್ನ ನಂಬಿ ಈ ಹುಡುಗ ವಿದ್ಯಾಭ್ಯಾಸ ಬಿಟ್ಟು ಮನೆಯಲ್ಲಿ ಸಾವನ್ನು ಕಾಯುತ್ತ ಕುಳಿತಿದ್ದ.ಅವನಿಗೆ ಆಗಿದ್ದಾದರೂ ಏನು ?ಸುಲಭವಾಗಿ ವಾಸಿಯಾಗಬಹುದಿದ್ದ ಕ್ಷಯ ರೋಗವಷ್ಟೇ.ಇದೇನೂ ಅಜ್ಜಿ ಕಥೆಯಲ್ಲ.೧೯೭೮ ಇಸವಿಯಲ್ಲಿ ನಡೆದ ಘಟನೆ.ನಾನಾಗ ವಿದ್ಯಾರ್ಥಿಯಾದ ಕಾರಣ, ನಮ್ಮೂರಲ್ಲಿದ್ದ ವೈದ್ಯರ ಮೂಲಕ ಅವನಿಗೆ ಚಿಕಿತ್ಸೆ ಕೊಡಿಸಿ ಕಾಯಿಲೆ ವಾಸಿ ಆದ ಮೇಲೆ ಅವನ ವ್ಯಾಸಂಗ ಮುಂದುವರಿದು ಇಂದು ಒಂದು ಪ್ರತಿಷ್ಟಿತ ಉಕ್ಕು ಕಾರ್ಖಾನೆಯಲ್ಲಿ ಉನ್ನತ ಹುದ್ದೆಯಲ್ಲಿದ್ದಾನೆ.ಆ ಹೋಮಿಯೋಪತಿ ವೈದ್ಯ ಮಹಾಶಯನ ಮಗ ಅಪ್ಪನ ವೃತ್ತಿ ಮುಂದುವರಿಸಿದ್ದಾನೆ.ಈಗಲೂ ಇಂತಹ ಸಲಹೆ ಕೊಡುತ್ತಲೇ ಇದ್ದಾನೆ ಅಂದರೆ ನಮ್ಮ ಜನರ ಹುಂಬತನ ಎಷ್ಟು ಎಂದು ಅಂದಾಜು ಮಾಡಬಹುದು.ಇವರಿಗೆ ಬಲಿಯಾಗುವುದು ಅವಿದ್ಯಾವಂತರಾಗಿದ್ದಲ್ಲಿ ಅರ್ಥ ಮಾಡಿಕೊಳ್ಳಬಹುದಿತ್ತು.ಆದರೆ ಅಲ್ಲಿ ಹೋಗುವ ರೋಗಿಗಳಲ್ಲಿ ವಿದ್ಯಾವಂತರೇ ಹೆಚ್ಚು.
ಪ್ರಸಂಗ ಎರಡು.
ನನ್ನ ಕುಟುಂಬದಲ್ಲಿ ಕೆಲವು ಪಾರಂಪರಿಕ ವೈದ್ಯರಿದ್ದಾರೆ.ಎಲ್ಲರೂ ಎಲುಬು ತಜ್ಞರೇ.೧೯೩೦ ರಿಂದ ಈ ವೃತ್ತಿ ಮಾಡುತ್ತಿದ್ದವರನ್ನು ನನಗೆ ನೋಡಿ ಗೊತ್ತು.ಆಗಿನ ಕಾಲದಲ್ಲಿ ಸರಿಯಾದ ವ್ಯವಸ್ಥೆ ಇಲ್ಲದ ಕಾರಣ ಎಲುಬು ನಿಜವಾಗಿಯೂ ಮುರಿತಕ್ಕೊಳಗಾಗಿದೆಯೋ ಎಂದು ನೋಡುವ ಪರಿಸ್ತಿತಿ ಇರಲಿಲ್ಲ.ಹಾಗಾಗಿ ಈ ವೈದ್ಯರು ಹೇಳಿದ್ದೇ ನಿಜ ಎಂದು ತಿಳಿದು ಚಿಕಿತ್ಸೆ ಪಡೆಯುತ್ತಿದ್ದರು.ಎಷ್ಟೋ ಸಂದರ್ಭದಲ್ಲಿ ವಾಸಿಯೂ ಆಗುತ್ತಿತ್ತು.
ನಾನು ವೃತ್ತಿ ಜೀವನ ಶುರು ಮಾಡಿ ಸ್ವಲ್ಪ ದಿನವಾಗಿತ್ತು.ನನ್ನ ಸ್ನೇಹಿತರೊಬ್ಬರಿಗೆ ನಾಯಿ ಕಚ್ಚಿತ್ತು.ಆಗ ಹೊಕ್ಕಳಿನ ಸುತ್ತೂ ೧೦ ಚುಚ್ಚುಮದ್ದು ಕೊಡಬೇಕಾಗಿತ್ತು.ಈಗಿನ ಹಾಗೆ ಕೈಗೆ ಚುಚ್ಚುವ ಇಂಜೆಕ್ಷನ್ ಅಲ್ಲ.ಅವರೇನೋ ನೋವಿಗೆ ಹೆದರಿ ಹತ್ತಿರದ ನಾಟಿ ವೈದ್ಯರೊಬ್ಬರ ಬಳಿ ಹೋಗಿದ್ದರು.ರಾತ್ರಿ ಸುಮಾರು ಎರಡು ಘಂಟೆಗೆ ನನಗೆ ತುರ್ತು ಕರೆ ಬಂತು.ಅವರು ಏನೇನೋ ಮಾತಾಡುತ್ತಿದ್ದರು,ಹುಚ್ಚರ ಹಾಗೆ.ಚಿಕಿತ್ಸೆ ನೀಡಿದ ಮಹಾಶಯನ ಪ್ರಕಾರ ನಾಟಿ ಔಶದಿಯಲ್ಲಿ ಮೊದಲು ಹುಚ್ಚು ಹಿಡಿಸಿ ಆಮೇಲೆ ಬಿದಿಸುವುದಂತೆ!ಸರಿಯಾಗಿ ನೋಡಿದಾಗ ರೋಗಿಗೆ ದತ್ತೂರದ ಸೊಪ್ಪಿನ ಚಿಕಿತ್ಸೆ ಕೊಡಲಾಗಿತ್ತು.ಈ ವೈದ್ಯರು ಏನು ಮಾಡುತ್ತಾರೆ ಗೊತ್ತೇ?ಮೊದಲು ದತ್ತುರದ ಸೊಪ್ಪು ತಿನ್ನಿಸುವುದು.ಆಗ ರೋಗಿಗೆ ಮತಿ ಭ್ರಮಣೆ ಆದ ಹಾಗೆ ಕಾಣಿಸುತ್ತದೆ.(ಹಾಗೆ ಎಲ್ಲರಿಗೂ ಒಂದೇ ಪ್ರಮಾಣದಲ್ಲಿ ಆಗುವುದಿಲ್ಲ)ಆದವರಿಗೆ ಹುಚ್ಚು ನಾಯಿ ಕಚಿದ್ದು,ಇಲ್ಲದವರಿಗೆ ಕಚ್ಚಿದ ನಾಯಿ ಹುಚ್ಚಲ್ಲ ಎಂದು ತೀರ್ಮಾನ ಮಾಡಿ ದುಡ್ಡು ವಸೂಲು ಮಾಡುತ್ತಾರೆ.ಆಮೇಲೆ ಅವರನ್ನು ರಾತ್ರಿಯಲ್ಲಿ ಆಸ್ಪತ್ರೆಗೆ ಸೇರಿಸಬೇಕಾಯಿತು.ಅಂತೂ ಕೊನೆಗೂ ಇಂಜೆಕ್ಷನ್ ಕೊಡಲೇ ಬೇಕಾಯಿತು.ಅಲ್ಲದೆ ಆಸ್ಪತ್ರೆ ವಾಸವೂ ಆಯಿತು.
ನಾನು ಖಂಡಿತವಾಗಿಯೂ ಆಯುರ್ವೇದ ಅಥವಾ ನಾಟಿ ಮದ್ದಿನ ವಿರೋಧಿಯಲ್ಲ.ಆದರೆ ನಾಟಿ ಮದ್ದಿನ ಹೆಸರಿನಲ್ಲಿ ಜನರನ್ನು ವಂಚಿಸುವ ಜನರ ವಿರೋಧಿ,ಅಷ್ಟೇ.ಈಗೀಗ ನಾಟಿ ಮದ್ದಿನ ಹೆಸರಲ್ಲಿ ಆಗುವ ಮೋಸ ನೋಡಿದರೆ ಎಷ್ಟು ಬೇಜಾರಾಗುತ್ತದೆ ಎಂದು ತಿಳಿಯಬೇಕಾದರೆ ನಮ್ಮ ವೃತ್ತಿಯಲ್ಲಿ ಇದ್ದವರಿಗೆ ಮಾತ್ರ ಸಾಧ್ಯ.ತೂಕ ಕಡಿಮೆ ಮಾಡಿಕೊಳ್ಳಬೇಕೆ,ಸಕ್ಕರೆ ಖಾಯಿಲೆಯೇ,ರಕ್ತದ ಒತ್ತಡವೇ.ಎಲ್ಲದಕ್ಕೂ ನಾಟಿ ಔಷಧಿ ಕೊಡುವ ಜನರಿದ್ದಾರೆ.ಆದರೆ ಅವರ್ಯಾರೂ ಪ್ರಸ್ತುತ ಇರುವ ಅಧುನಿಕ ಪದ್ದತಿಯ ಚಿಕಿತ್ಸೆಯನ್ನ ಬಿಟ್ಟು,ಔಷಧಿ ಕೊಡುವುದಿಲ್ಲ.ಹಾಗಾದರೆ ಅವ್ರು ಯಾಕೆ ತಾನೇ ಚಿಕಿತ್ಸೆ ಕೊಡಬೇಕು?

Imagine a world with no modern medical science no matter what is said against it.
Had Edward Jenner not discovered vaccine against smallpox hundreds of thousands would have died of the dreaded disease every year.Imagine a world without Alexander Fleming's discovery of the antibiotic Penicillin!(we may argue about India having ideas about inoculation against diseases or about the soil containing some thing with which infections could be cured(look at Wikipedia,i can provide link if any one is interested)
Whether it is antibiotics or vaccines , insulins, anti hypertensives, hormones,antivirals or the surgical procedures,the list is endless for the invention of which we stand at the feet of modern medical science.
Since the beginning of human existence in India thousands of years ago till about 1930
the population of India was about 300 millions.It jumped to 1.2 billions in 2009 in spite of the population control measures.
The most sensitive health index-the infant mortality rate which was around 345 per 1000 live births in 1940 came down to around 15 to 25(lowest in Kerala and higher in states like Bihar).Maternal mortality came down to less than one per thousand deliveries.Diseases like Polio were almost eradicated with smallpox being totally eradicated from the face of the earth.Life expectancy at birth increased from 40 years in 1940 to 67 years today.It is no coincidence that this happened as the modern medicine developed the above mentioned tools like drugs and surgical procedures.
The much maligned Cesarean operation has reduced the incidence of would be mother dying at delivery to almost zero.And the greatest culprit of all as the various reports in the media would suggest,cardiac surgery has increased the quality of life of people who have had cardiac problem.
One may claim diseases like diabetes and heart ailments are on the increase.Yes,that is not because of medicine but because of reason beyond the control of doctors in the sense that the people themselves are responsible.Changed lifestyle is the primary reason for this spectacular increase.But it is still modern medicine that can do something to keep these under control and the patient from harms way.
I know defenders of alternate system of medicine will be at my throat for this,but let me assure you, without modern medicine which I refuse to call allopathy,we would be miserable.
There will be this matter to argue-modern medicine is too commercial.Yes,then which system is not?The basic idea behind practicing what you study is to earn a living.Just because I happened to study medicine i can not go for social service with no concern for my stomach.One can not work on empty one.
Whenever a new disease is discovered the market is flooded with cures from quacks and from alternate systems.And the discovery is by modern medicine and the miracle cure from the alternate system!If this is not because of commercial reason then what is? At least modern medicine is held responsible for commissions and omissions,alternate system is not.
Recently in this part of India we had an epidemic of chikungunya.We,practitioners of modern medicine know exactly what it is and how it can be managed.But no,we need this half baked systems telling us how to manage that particular problem!This year we have some cases of viral fever and again the press and these very same people are at it again.
OK,we have no proper cure for each viral disease(We do have anti viral treatment for many a virus) but if any branch of medicine can handle it modern medicine can.For the simple reason that it is the discovery of this system.
The so called defenders of Indian system including some Swamijis talk about the superiority of Indigenous medical practice being safe for all ailments!Without basic understanding of the issues involved they advocate the methods that suits their ulterior motives. And do you believe any medicine is free from side effects?Don't,because above normal even daily food has side effects.Try eating a couple of hundred grams of the commonly used Garlic!Or even quarter Kg. of sugar!
Do you know most of these practitioners use drugs like antibiotics and alarmingly, very potent steroids,of which they hardly have any knowledge, to achieve dramatic cures in all these viral disease?Isn't there any adverse outcome?Certainly there are but they go unreported!The reason is simple,
You know you have been foolish enough to approach quacks or people who are not supposed to use these drugs and you will hardly admit it was disastrous.And the person responsible is no way harmed.
In contrast if the outcome is adverse if a qualified person is involved people gang up and the person is harassed even if he did everything that could be done.People jump into concluding how greedy and careless the doctor was and so on and so forth.Newspapers immediately jump into the fray to condemn the person involved.What do these people know about modern medicine?Nothing.But they do have an opinion no matter what.
But ultimately it is the modern medicine that has the last laugh because it is the only system that tries to understand diseases.Modern medicine has description if not cure for all ailments.
The so called practitioners of alternate system study it not because they want to, but because of their inability to study modern medicine as the opportunity in India is limited.To establish a modern medical school it requires an elaborate infrastrucure in contrast to alternate system.The so called institutes teaching alternate systems are so numerous I wonder from where they even get faculty to teach!
Think about a patient with chest pain going to an Ayurvrdic physician.He is immediately sent to a cardiologist.That is when common sense should dictate that you go to a qualified modern medical practitioner for your needs.And if he decides it is nothing serious one can get treated by any system.
The standard explanation handed out by these alternate system is that for emergency modern medicine is needed and then they can take over.Why?Once the emergency is past there is no need for other system to take over.
In fact the proper alternate is that one should practice what is taught during studying and nothing else.And people with no qualification should be barred from declaring themselves"doctor"
The common man has to be vigilant.

basic idea
There was a time when the family used to visit a GP for all medical advise and treatment.the doctor used to be an elderly kind and knowledgeable and most importantly,dependable.
the times have changed.life today is on the fast lane.work can not wait,whether one is sick or healthy.there is a deadline.
once people get literate(differentiate from literate) they think they have right to express their opinion on every topic.they are right though.but when getting an opinion from a professional is concerned lay opinion has no place.
there is an oft repeated complaint in India that medical profession is no longer what it was and that it has gone really high tech.that doctors do not use their clinical acumen.partly true.
what about the patient?
Today people are too demanding.they have unrealistic ideas.
In India the alternate system also is very popular.but most of those practitioners do not practice their system with sincere devotion leading to a mixture of systems which is not good in the long run.
will write more before you start commenting which mostly will be hostile because i know i am treading over too many feet.

Approach
The answer is not hard to guess.
The origin of modern medicine is rooted in Ayurveda which was taken out of India and then they worked on the knowledge gained from there.And why not,knowledge should benefit the public,not any individual.
And hence developped the modern system of medicine which was standardised.New knowledge developped due to the continued reaserch carried out by the western scientists which they are doing even now.(to a certain extent scientists from other countries)
I have nothing against any system of medicine so long as it benefits mankind.Unfortunately research in alternate system of medicine is limited to say the least for whatever reason.Then how to decide which system to opt for?
In any ailment the primary concern has to be the proper diagnosis.Unfortunately for this investigations are must.Looking at the patient no physician can rightly diagnose unless it is a minor ailment which will be cured without any help from the physician.On the other hand if it happens to be anything serious, who is to diagnose?
Let us face the facts,right now we have this fecility only in modern medicine.And one has to go for it,whether he likes it or not.After the fact,
who is to interpret the results?Naturally a person qualified in the perticular system and after that has been settled comes the treatment part.
If one decides to go for any other form of treatment he should be certainly given a choice with clear explanation of the consequences should the choice of the patient is not suitable for the desease.But can the patient decide the future course of treatment?I think not.That is where
the primary care physician should intervene.(Regarding primary care physician in next installment.)
Primary Care
Medicine is one field where one will regret such action because what is good for me need not be so for the other person.
Every one of us have visited a doctor in our life. In the past,it used to be a local GP.Whatever he used to advise was like scripture.He used to be a friend,philosopher and guide as described in text books of medicine. Slowly we advanced and had all types of specialist doctors in our midst,that lay public started deciding whom to consult,which in the past used to be decided by the local family doctor.This is when things started to go wrong.
I am not suggesting one should not go to a specialist but before that his local doctor should be consulted and preferably give a reference so that you go to the right person and he has some idea as to from where to start,else if you approach a wrong specialist he may not be in a position to do justice to the case.
For example take a case of pain abdomen(stomach ache).It may just be just an ordinary pain or serious ailment like appendicitis where early intervention by a surgeon will save lot of complications,both physical,psychological and financial.
I get all types of questions,from people who have been coming to me for all their health needs. Children become adults and take a job in a new place where they feel lost.They find it difficult in a new place when it comes to the selection of a primary care physician.It is easy really.You may be wondering how so.Let me explain-when you need to see a doctor,go to any qualified person and explain your problem,on the other hand you can ask your previous doctor for a recommendation(many a times it so happens that your family doctor knows someone there).Make it very clear to him that you do not expect any miracles,that you are ready to undergo required tests to felicitate diagnosis and that you are ready to spend reasonably.Because we doctors encounter all sorts of people in day to day practice(it has been my experience-many people simply do not want to pay the doctor assuming he has made enough to need anything more!)Many diseases can not be clinically diagnosed and need some clinical tests.And when a doctor orders those tests people think they are being fleeced.Hardly so,because the doctor expects the patients back!And finally when he does prescribe something ask him whether you really need those medicines.Now you will tell me the doctor is awfully busy to explain,never so.On the other hand he will and should be pleased because too much expectation by the patients make the doctors to overprescribe.And if the doctor concerned is really annoyed please tell him politely that it is your health you are worried about.It is your right and his duty.And never go to him again.There are bad apples in every barrel.
One thing every one of us should understand is that there is no fast cure.It depends on the disease,and one particular doctor can't do any better than the the other.And remember there is no medicine that is "strong",medicines are either necessary or not.If necessary,however strong it may look, it is not.
You may be busy but so is the doctor.If you are in a hurry and tell the the doctor that you have no time to rest, he may not be able to help you.If you want proper care you got to spend some time and money,after all it your health.

Most of the patients do not realize this basic fact.Of course they don't need to because they are sick and that is the reason they go to doctors.True.
Once I was eating lunch at a restaurant.An educated man who is a regular of mine joined my table and started showing an infected wound full of pus right there!I am a doctor,no arguing about it but I am a human being too,and I too have sense of hygiene and like to eat in a clean surroundings.He had done the damage.It was a no win situation,protest I am damned keep quiet and the same.I stopped eating and even today if I remember that incident while eating I retch.Please have mercy.
A doctor attends a function like any other man,do not convert the wedding hall into a consulting room.Whenever you have a problem ask him when he will be available.Now that communication is no longer a problem that is the least you can do.
No doubt you pay for his service but remember that you don't own him.
After you think you are well,report your status,for he may continue your treatment for some more days and it is only reasonable that he may want to know what happened with his treatment.
Know his timings. Go well in advance so that he may get out at his appointed hour.It is another matter in emergencies.
I have seen patients coming to me during my lunch hour and claim they were in the vicinity and so dropped in!
If you have a doctor as your neighbor don't go and knock his door at inconvenient times for small problems.Ask him beforehand whether he will see you at home.By going to the house of a doctor except in emergencies puts the doctors' family in danger of catching your disease if it is indeed communicable.I always get mortally afraid when a person with communicable disease comes to my house,especially during examination season as i too have a college going son!
If possible wish your family doctor on important days because he is the last person to come to any one's mind except during an illness.
If you consider your doctor as a human being first,I think he surely will reciprocate and it will be mutually beneficial.
